2/13/2023 EnchantaVaughn Approved Behavior: Upon arrival to the home setting, Client was quiet upon arrival of counselor. She was excited to see the counselor as evidenced by smiling and asking her about her day. Client shared that she has been sad lately but did not know why. Client shared that she needed help on building more confidence like those around her have. Client expressed that she was frustrated but could not completely communicate her feelings. Behavior: Client expressed that she needed to talk with counselor about a few things to help her figure stuff out. Client shared that she continues to struggle with communicating with those around her. Client shared that she is becoming anxious and does not know why. She stated that she was ready to share her feelings with mother even though she knows this conversation has occurred before. Client was excited to begin the session and eager to discuss what she believes some of her internal issues are. Intervention: QMHP-C utilized cognitive skills to give Client a chance to openly and honestly discuss her feelings about her current state. QMHP-C utilized active listening skills and was empathetic with Client as she vented the overwhelming events that had occurred in school. QMHP-C reiterated the importance of positive thinking and exercising her coping skills. QMHP-C prompted Client to address and respond to Mom with respect and be thankful for her support. Intervention: QMHP-C redirected Client's negative attitude by reminding her that she should not expect immediate gratification upon her request. QMHP-C highlighted the faulty and unnecessary behavior that Client was engaging in. QMHP-C educated Client that there are different ways to handle conflicts. QMHP-C reminded Client not to take everything personal. QMHP-C explored Clientâs capability to cope with his stressors and to get back on track. Response: Client was observed and reported to have been pleasant and cooperative during psychoeducational activity and demonstrated a willingness to adhere to rules and expectations. Counselor praised Client for being on her best behavior and correcting her mistakes. Response: Client displayed good behaviors, raising her hand when she asked a question, waiting for her turn to speak, using her words instead of hand signals, saying please and thank you, and giving her counselor good eye contact. Client said she felt great and wanted to color a picture. Client utilized deep breathing techniques to relax and stay calm during stressful environment. Client administered positive behavioral expectations, reducing ADHD.
